Becoming a patient at Medisch Huis ’t Rondpunt

To become a patient, it is necessary to register in the fixed-rate system. This system facilitates direct billing between your health insurance fund and Medical House ’t Rondpunt. As a result, you don’t need to pay for consultations or home visits yourself, nor do you need to submit documents to your health insurance fund.

Our nursing care and social services are also included. However, make sure your registration with the health insurance fund is in order!

How to Register for the Fixed-Rate System

1. Download the registration form. This form is an agreement between you, the health insurance fund, and our practice, confirming your registration.
2. Print the form twice. (Printed forms are also available at our reception desk.)
3. Read the contract thoroughly. If you have any questions, our reception staff is available to assist you by phone or at the desk.
4. Complete both copies of the form (page 2): Fill in your name, first name, date of birth, gender, address, phone number, the name and address of your health insurance fund, and your national registration number.
5. Attach stickers from your health insurance fund to both copies of the form.
6. Sign both documents, including the date and your name.
   For minors, all personal information must be completed for the child, along with a contact phone number for the parent/guardian. The documents must be signed by the parent/guardian, including their name and national registration number.
   For minors aged 14 to 18, the minor can fill in and sign the document themselves.
7. Submit the documents to the practice as soon as possible. Bring your eID (electronic ID card) so we can scan it. This ensures our electronic system links correctly with your health insurance fund.
8. Our reception staff will register your form, and it will automatically be sent to your health insurance fund.
9. At the reception desk, you’ll also sign a form authorizing us to request your medical file from your previous doctor.
10. You will receive one signed copy of the agreement back from Medisch Huis ’t Rondpunt.

This consultation is also valuable for our doctors. Your file will be fully updated, enabling them to work in an informed and effective manner.
